Elevated TMAO levels have emerged as a powerful predictor of cardiovascular risk—even in people who seem otherwise low-risk.
How Does TMAO Impact Your Heart?
When TMAO levels are high, it can disrupt how your body manages cholesterol:
- Impaired Reverse Cholesterol Transport: TMAO interferes with the process that removes cholesterol from your artery walls.
- Cholesterol Accumulation: This disruption can cause cholesterol to build up, leading to the formation of foam cells and arterial plaque.
- Increased Inflammation: The result is greater vascular inflammation and a higher risk of atherosclerosis, heart attack, and stroke.
Clinical studies have shown that people with the highest TMAO levels are up to 2.5 times more likely to experience major adverse cardiovascular events over three years. Worryingly, TMAO can predict risk even in those under 65, non-smokers, and people with healthy LDL cholesterol.

You can lower your TMAO by shifting to a Mediterranean-style diet—rich in fruits, vegetables, whole grains, and healthy fats—which supports a more diverse, balanced gut microbiome. Vegetarians and vegans typically have the lowest TMAO levels, highlighting the impact of dietary choices.
