If you are unable to print this 8-page Intake Form, please arrive 30 minutes prior to your appointment or call the office a week in advance to arrange for the form to be mailed to you.

  Use this form to request medical providers to send your records to Boise Natural Health Clinic. Please do not ask for your whole chart to be sent to us. Usually, we only want Lab/Test Results to start with. Another request can be made later if more is needed.
